Can an employee opt into a specific plan and switch to a different plan later on?

No, the employee must remain in the plan chosen by the Customer. For countries where it is mandatory to offer this benefit, if the Customer doesn’t choose a plan, the employee will be enrolled in the Standard plan by default.
