In this article, we explain how to download your EPF passbook and EPFO service history and share them during onboarding in India.
Why this is required
To confirm whether Employees' Provident Fund (EPF) and Employees' Pension Scheme (EPS) apply to you, we review your EPF passbook and service history.
This information helps us:
- Verify whether EPF and EPS contributions are applicable
- Ensure your Form 11 is completed accurately based on the EPF and EPS applicability.
- Avoid delays in your onboarding and payroll setup
Before you start
To access these documents, you need:
- your Universal Account Number (UAN)
- your EPFO portal password
- access to your registered mobile number for the one-time password (OTP)
If you forgot your password, select Forgot Password on the EPFO member portal and follow the steps to reset it.
How to download your EPF passbook
- Log in to the EPFO member portal using your UAN and password
- Complete OTP verification using your registered mobile number
- Navigate to the “Passbook” section
- Download your passbook or take a screenshot showing your last five contributions

How to download your EPFO service history
- Log in to the EPFO member portal using your UAN and password
- Complete OTP verification using your registered mobile number
- Navigate to the “Service History” section
- Download the PDF and save the file

How to review your service history for Form 11
Check the Date of Joining (DOJ) listed under EPF and EPS in your service history.
- If a DOJ under EPF appears for any previous employer, select Yes for this in Form 11 question: “Whether earlier a member of Employees' Provident Fund Scheme, 1952”
- If a DOJ under EPS appears for your first employer, select Yes for this in Form 11 question: “Whether earlier a member of Employees' Pension Scheme, 1995”
- If no DOJ under EPS appears for your first employer, select No for the Employees' Pension Scheme question in Form 11.
This check helps us confirm whether EPF and EPS deductions were made in your previous employment and ensures your onboarding details are accurate from the start.
What to submit
Please share:
- a screenshot of your EPF passbook showing your last five contributions
- your EPFO service history PDF
Make sure both files are clear and readable before you upload them to the Remote platform under Files.
